Butler               Monday, Oct.16    4 p.m. - 7 p.m.          Courtroom
Butler Clinic Cancelled due to lack of vaccines
Bloomingdale Monday, Oct. 23   4 p.m. - 7 p.m.          Firemen's Hall
Kinnelon          Thursday, Nov. 9   4 p.m. – 7 p.m.         Borough Hall
Butler                Wednesday, Dec 13 4pm - 7pm           Courtroom

Clinic criteria:
•Must be resident of Butler, Bloomingdale or Kinnelon
•Over the Age of 18
•Children with chronic illness: age 9-18 years with a doctor’s note; age 4-9 years with a doctor’s note may be eligible only if they have received a flu shot prior to this year (Any eligible child receiving vaccine must be accompanied by a parent or guardian.)
