Butler Dog License Application

Dog License Renewals begin each year in January.
Dog License Fee: $10.00  Non-Spayed/Neutered $13.00
NOTE: Starting March 1st. a $5.00 late fee will be added to dog license fees making them $15.00 and Non-Spayed/Neutered $18.00.

NOTE: Copies of Rabies Vaccination OR Rabies Exempt form signed by a vet AND proof of Spay/Neuter MUST be uploaded with this application. (Spay/Neuter only needed for initial registration)
(RABIES VACCINATIONS MUST BE VALID THROUGH OCTOBER 31st.)

If you no longer have the dog, please exit this form and complete a PET LICENSE CANCELLATION form.

* A dog license is required for all dogs starting at 6 months old.

In order to complete this application you must:

1. Upload a proof of your dogs Rabies Vaccination or Rabies Exempt Form, signed by your vet.
